Abstract
In this chapter, the author discussed the advent of accountable care organizations (ACOs) and the different types of ACOs that exist. A comparison is made to HMOs and Medicare Advantage programs. There is a discussion on how the incentive for hospital admissions is juxtaposed with incentives to keep patients out of the hospital. The opportunity to develop an inpatient ACE (Acute Care of the Elderly) unit is discussed. The need to successfully interact with skilled nursing homes is also addressed in relation to preventing hospital admissions and readmissions. Finally, the concept of delivering consistent care along the continuum of care is discussed.
