Abstract

Pancreatic cystic lesions (PCL) are relatively rare and in recent years they are being increasingly recognized with the improvement and widespread use of cross-sectional imaging tools. The diagnosis, differentiation, and management of PCL are mandatory since they may carry malignant potentials. PCL are usually diagnosed incidentally on conventional imaging tools including ultrasonography, computed tomography, or magnetic resonance imaging; however, these modalities still have a limited ability to differentiate a benign lesion from malignant. With the high-resolution capacity and better imaging characteristics, endoscopic ultrasonography is superior to these conventional methods. Endoscopic ultrasonography guided fine-needle aspiration provides biochemical, cytological, and DNA analysis of the cystic fluid, which might better determine the lesion. Surgical resection, endoscopic treatment, and follow-up conservatively are the options for treatment. Further diagnostic tools and techniques are still needed to resolve the gray zones in the management and treatment of these lesions.
