Abstract
Enteric leaks following Roux-en-Y gastric bypass cause devastating morbidity and are one of the main causes of mortality. The most common presenting signs and symptoms include abdominal pain, tachycardia, and fever. Upper gastrointestinal series or computerized tomography scans can be used to make the diagnosis when the clinical picture is equivocal. Prompt diagnosis and treatment optimizes outcomes. Preventative measures include meticulous dissection, ensuring adequate blood supply and minimal tension. Buttressing materials can be used to reinforce staple lines. Treatment is largely surgical for source control, debridement, and possibly placement of a gastrostomy tube in the gastric remnant.
