Multivessel Disease in the PCI Era: Where Is CABG?

Abstract

Coronary artery bypass grafting (CABG) and percutaneous coronary intervention (PCI) are well-established invasive therapeutic options for coronary revascularization. Significant advances in both these therapeutic modalities, particularly PCI technology, have generated a major debate about the superiority of one modality over the other. The last decade has seen the emergence of PCI as the preferred revascularization modality in patients with single-vessel or low-risk multivessel disease. PCI is also being increasingly offered to treat complex multivessel disease in patients with increased risk of adverse surgical outcomes. On the other hand, CABG, that had been the predominant mode of revascularization for almost five decades, is now the preferred strategy for patients with complex multivessel disease and unprotected left main stem disease, especially those with diabetes mellitus and left ventricular systolic dysfunction. This chapter evaluates the best available evidence to establish the current status of CABG for the management of multivessel coronary artery disease in the PCI era.
