Abstract

Rheumatoid arthritis (RA) is a chronic disease characterized mainly by joint inflammation that can lead to permanent structural damage. An accurate assessment of joint inflammation is of outmost importance for disease management. Over the past years, musculoskeletal ultrasound (MSUS) has proved to be a sensitive tool not only in detecting joint and periarticular inflammation, i.e., synovitis, tenosynovitis, and bursitis, but also in evaluating structural damages such as erosion and tendon damage. In addition to clinical and laboratory evaluation, MSUS has been successfully used to early diagnose RA. In RA patients, B-mode (BM) and Doppler techniques characterize better than clinical assessment joint inflammation (either synovial hypertrophy or effusion, either active or inactive synovitis) and reflect accurately disease inflammatory activity. Doppler-detected synovitis has been shown to be predictive for structural damage, even in patients in clinical remission. Furthermore, MSUS has proved successful in monitoring response to treatment and is also a valuable tool for guiding MSUS injections.
