Chronic Kidney Disease and Renovascular Interactions

Abstract

Chronic kidney disease (CKD) and end-stage renal disease (ESRD) confer a markedly enhanced risk of cardiovascular diseases. Cardiovascular complications which result from exposure to a wide range of cardiovascular risk factors (traditional and uremia related) and wide spectrum of vascular pathologies are the major cause of cardiovascular morbidity and mortality. The vascular pathology which involves both the macrocirculation and microcirculation is characterized by atherosclerosis, arteriosclerosis, remodeling of the precapillary resistance vessels, endothelial dysfunction, and cardiomyopathy. The natural history of the vascular lesions appears to be age related. In pediatric CKD/ESRD, exposure to the uremia-related risk factors leads to the development of arteriosclerosis and cardiomyopathy. With increasing age and the appearance of traditional risk factors, atherosclerotic complications become prominent.
