Abstract
In this chapter, the author describes a new minimally invasive surgical technique for fusing and fixating the sacroiliac joint via a posterior inferior approach. The technique permits minimal muscular and ligamentous disruption while simultaneously permitting direct and extensive cartilage removal and decortication of the articular portion of the joint in order to enhance fusion potential. Subsequent to joint preparation, intra-articular implants are positioned into the joint space and supplemented by percutaneous lateral fixation. Multiple case studies are presented. To serve as a background to the evolution behind the new procedure, the author reports on his personal experience of surgically treating hundreds of painful SIJs with varying techniques along with the results of an open SIJ fusion study presented in the 1990s demonstrating significant improvement of patients’ SIJ pain with long-term follow-up demonstrating durability.
