Abstract
One of the results of clinical research is direct benefit to patients, communities, and society. With scarcity of healthcare resources, allocation of funds and incentives for clinical research needs to be prioritized. For a variety of reasons, clinical trials have traditionally excluded large segments of society, including ethnic minorities, women, children, and the elderly. In some cases, this has led to neglect of important diseases and subpopulations. In other cases, this has led to direct harm because of unforeseen biological differences among certain groups of people. The composition of society is changing, and resources need to be distributed equitably to reflect demographic trends. Regulatory agencies, community groups, and political leaders are promoting research which addresses these gaps.
