Abstract
It is commonly believed that hormonal contraception has significant effects on a woman’s mood. However this belief has not been well substantiated and the literature is replete with conflicting data. Inconsistent research findings echo the frequent clinical observation that patients experience disparate reactions to hormonal contraception, making it challenging to medically manage and counsel patients in their use of hormonal contraception in regard to their mental health. The relationship between exogenous hormones and mood disorders has not been studied in sufficient depth to provide a definitive answer to the question of whether these hormones have a direct effect on clinical psychiatric outcomes. This chapter will guide clinicians towards making the best contraceptive choices for their patients who have, or are at risk for developing, mood and anxiety disorders. This chapter will review the neurobiological basis for the interaction between exogenous hormones and mood; outline the indications, risks, and common side effects of currently available hormonal contraceptives; and review the scientific literature to guide recommendations for identifying the ideal contraceptive method for a given patient in a patient-centered, individualized process that accounts for their mental health history.
