Abstract
At menarche, the incidence of migraine in girls increases. Migraine also changes at other key times in a women’s life: during menses, with the use of oral contraceptive therapy, and with pregnancy, lactation, and menopause. Each of these hormonal milieus is discussed in this chapter with relation to headache.
This chapter includes sections on diagnosis of menstrual migraine, followed by discussion of acute, preventive, and mini-prevention strategies. The impact and controversies of contraception in female migraineurs are considered, with a special discussion on stroke risk. An extensive section is included on migraine during pregnancy and lactation, with emphasis on practical treatments. The chapter ends with clinical pearls on treatment during perimenopause and menopause.
