Verschiedene biologische Faktoren sind assoziiert mit der Entstehung des peptischen Ulkus. Die relevantesten davon sind die Infektion mit dem Bakterium Helicobacter pylori und die Einnahme von non-steroidalen Antirheumatika. Bei 5–20% fehlen aber identifizierbare Ursachen. Bei diesen, aber auch bei allen anderen Ulkuskranken, mögen psychosoziale Faktoren für die Entstehung des Ulkusleidens eine bedeutende Rolle spielen. Neueren Studien zufolge wird geschätzt, dass Stress bei 30–65% der Betroffen zur Ulkusentstehung beiträgt. Im folgenden Kapitel wird die bisherige Forschung zu psychosozialen Faktoren, insbesondere Stress, als mögliche Mitverursacher für das peptische Ulcus kritisch dargestellt.
