Das Gallengangskarzinom ist mit 3 % aller Tumoren eine seltene Erkrankung. Es kann an jeder Stelle des Gallengangssystems, von den kleinsten peripheren intrahepatischen Gallengängen bis zur Papilla vateri entstehen und wird auf Grund der Lokalisation in drei Subtypen unterteilt: Das intrahepatische cholangiozelluläre Karzinom (CCC, s. auch Kapitel 15), das hiläre oder zentrale Gallengangskarzinom (Klatskin-Tumor) und das distale Gallengangskarzinom. Das Papillenkarzinom stellt eine Sonderform dar und wird hier nicht weiter berücksichtigt. Histologisch handelt es sich bei den Gallengangskarzinomen in 90 % um Adenokarzinome. Andere Formen sind adenosquamöse Karzinome, Plattenepithelkarzinome, kleinzellige Karzinome und undifferenzierte Karzinome. Die Inzidenz variiert stark, je nach geographischer Region und Geschlecht. Sie stieg beim intrahepatischen und extrahepatischen Gallengangskarzinom in den letzten Jahren an. So liegt die Häufigkeit des intrahepatischen Gallengangskarzinoms in Nordost-Thailand bei Männern doppelt so hoch wie bei Frauen. Auf 100 000 Einwohner erkranken dort 96 Männer und 38 Frauen. Dagegen werden in Australien auf 1 000 000 Einwohner nur 1 bis 2 cholangiozelluläre Karzinome diagnostiziert. Bei den extrahepatischen Gallengangskarzinomen wird je nach Region weltweit eine Inzidenz von 0,5 bis 1 Erkrankung auf 100 000 Einwohner angegeben.
