Abstract
Here we begin to delve into the heart of the book, deep brain stimulation and some of the mysteries that continue to confound the field of neuromodulation. Topics covered include: history of DBS, which patients are good candidates for this procedure, different options available to patients, the options surgeons have for electrode placement, and when patients should consider getting DBS.

One example is the pedunculopontine nucleus (a.k.a. PPN) which had been hyped up, but most researchers believe that it is not as effective in treating gait and balance as initially thought. Another is the Globus Pallidus pars enterna (GPe) which has been proposed as a target to improve bradykinesia, although preliminary results are not that encouraging. The Nucleus Basalis of Meynert has been stimulated with the aim to improve cognition in PD patients but, once again, the effects—if any—are very modest.
