Abstract
Anterior and lateral approaches in the revision setting for lumbar spine surgery can be a highly effective way to address spinal pathology. However, due to scarring, altered anatomy, and obliterated surgical planes, this undertaking presents unique risks and challenges. The decision of which side (right versus left) or which surgical plane to utilize (retroperitoneal, transperitoneal, or direct lateral) is essential when planning a revision surgery. This chapter reviews the critical concepts regarding which approach is appropriate and how to best avoid and manage complications in the revision setting. Close coordination with a surgical team, consisting of a spine surgeon, access surgeon, endovascular surgeon, urologist, and anesthesiologist, can lead to safe and effective revision surgery from an anterior or lateral approach.
