Abstract
The direct lateral transpsoas approach can be a safe and powerful technique for multiple indications in the lumbar spine. A comprehensive knowledge of this technique is critical to maximize effectiveness and minimize patient risk. Numerous factors, including preoperative planning, patient positioning, retractor selection, instrument specialization, radiology technician experience, and general workflow play a role in limiting time spent within the substance of the psoas and time spent retracting the lumbosacral plexus. The purpose of this chapter is to discuss indications, contraindications, advantages, disadvantages, and surgical considerations when undertaking a direct lateral transpsoas approach to the lumbar spine.
