Abstract

Minimally invasive surgery is now the standard in most surgical specialties, with single-position surgery and mini-open techniques trending in the spinal literature. There still remains, however, a role for more conventional open techniques, based on patient anatomy and surgical experience, utilizing the readily available retractor systems. Minimally invasive retractor technology also utilizes fixed vascular retraction, as opposed to intermittent vascular retraction more commonly used with conventional open approaches, the latter offering a potential patient safety advantage. Although cosmetics and potential decreased surgical morbidity are important considerations, risk mitigation should be the primary focus of the surgeon when selecting a retractor system.
