Flexible Approach Tympanoplasty/Mastoidectomy: A Pathology-Guided, Pathogenesis—Oriented Surgery for the Middle Ear

Abstract

Preoperative evaluation remains the cornerstone in the determination of an appropriate surgical procedure for patients with otologic disease. Clinical microscopic examination, sophisticated audiometric evaluation, and CT scanning and MRI techniques all suggest the exact dimensions of pathologic conditions in the ear. As every surgeon understands from experience, surgical exposure of pathologic tissue usually provides a better, more thorough understanding and more information than preoperative estimation of disease. Surgical techniques should fit the pathology, as it is understood intraoperatively. The flexible approach has been found to be adaptable to many and various forms of pathologic conditions found in the temporal bone. These include inflammation or infection, congenital stenosis, benign and malignant tumors, and traumatic injuries.

Among all pathologic findings in temporal bone, this approach finds its best indication in the surgical treatment of otitis media (OM) and its sequel.
