Tympanoplasty: Underlay Technique Step by Step and the Complex Tympanoplasty

Abstract

As a follow-up of the chapter “Tympanic Membrane Perforations: New Profiles, New Strategies,” which brings a new perspective regarding the classification of tympanic perforations (with inferences about their pathogenesis), we present in this chapter an illustrated step-by-step surgical description. The retroauricular approach is preferred by our group since we consider it a “horse battle” in surgical otology, as it can adapt to a series of situations that arise while performing the surgeries. Thus, we first detail and illustrate the underlay retroauricular tympanoplasty with the interposition of the temporal fascia alone, which we call the “simple” tympanoplasty. Later on in the chapter, we discuss additional steps that some types of perforation may require, thus characterizing what we call the “complex” tympanoplasty.
