Home Care for Older Adults Using Interprofessional Teams

Abstract

Medical social workers have comprehensive roles in providing home care services to patients who are older and working collaboratively with other professionals. There are many ways to use the home environment to strengthen traditional assessment procedures. While there are a multitude of factors that must be considered by medical social workers in the home care milieu, safety concerns and crisis planning are two integral features. The provision of telehealth services is valuable for older patients in need of home care services. The practice of social work is guided by the person-in-environment theoretical framework which describes how people are influenced by their environment.

Assignment: “Make a Plan” Activity

    Medical social workers are critical in assisting seniors and their caregivers in the disaster preparedness and response plan. Use the provided emergency preparedness checklist to create your emergency plan.

    Completing this “make a plan” activity will help you know what to do in an emergency. It will take you about 20 min to make your plan. Your family may not be together when an emergency happens. Plan on how to meet or contact one another and discuss what you would do in different situations. Finally, keep this document on an easy-to-find emergency kit in case of an emergency.
