Treatment of Small Cell Lung Cancer

Abstract

Small cell lung cancer (SCLC) is a neuroendocrine tumor strongly associated with tobacco use, encompasses ~12% of lung cancer diagnoses, and is systemic even at early stages. Local therapy alone, such as surgery or radiation, is insufficient in long-term control, even early-stage disease. Consequently, a backbone of systemic therapy with platinum- and etoposide-based chemotherapy has been a standard of care with modest advances until the last 5–10 years. In the chapter, we explore diagnosis, staging, and clinical presenting symptoms of SCLC. We also discuss optimization of local therapy with timing and radiation dose in limited-stage disease. Novel therapies are improving outcomes in extensive-stage disease, and we discuss these new treatment approaches as immunotherapy. We also explore systemic therapy options in relapsed SCLC. Further, we explore the risk of CNS metastases and the role of prophylactic cranial irradiation (PCI). Finally, we discuss novel therapies, including hippocampal avoidance PCI.
