Abstract
Patients with coronary artery related chest pain have a wide range of chronic pathologies. They may have a history of single vessel disease which was successfully treated or may have a history of CABG, stenting, and disease that is not amenable to further intervention. The entire coronary history and timeline is significant. A drug eluting stent may have been placed 1 year ago or one decade ago. This leads to different considerations when evaluating the symptomatic patient. Medical therapy instead of repeat intervention may be the best therapeutic option. The patient may have exertional chest pain without epicardial CAD. This is called microvascular dysfunction (MVD). Medical management is necessary to improve exercise capacity and reduce ischemic symptoms in these groups of patients. Secondary prevention measures with cholesterol lowering agents are essential for improved mortality.
