Appendicitis: Somatic and Visceral Nerve Reflexes Signs: Mechanisms and Pathophysiologic Approach in Acute and Chronic Disease

Abstract

Appendicitis is an acute or chronic inflammatory condition of the vermiform appendix. It is a common cause of acute abdominal pain, particularly in children and young adults. Patients present with various subjective and objective symptoms depending on the stage and severity of the disease. Prior to the advent of advanced imagining techniques, the diagnosis was entirely clinically based, prompting physicians to devise a variety of maneuvers, eponymously named as a “sign” to honor their contribution as a method of diagnosis. These maneuvers involved the activation of visceral and somatic nerve pathways causing sensory and motor phenomena. Seventy-four eponymously named signs are ascribed to acute, subacute, and chronic appendicitis. A thorough understanding of the principles, mechanisms, techniques, and pathophysiologic processes allows clinicians to account for the location of the disease, pain, and characteristics finding involving the abdominal wall, iliopsoas, and obturator muscles. These features further help support, in addition to the patient’s history, the clinical diagnosis of this disease.
