Adult Metacarpal Base and CMC Dislocations Fractures

Abstract

Metacarpal base and carpometacarpal (CMC) fracture-dislocation injuries of the second through fifth metacarpals can represent a challenging hand injury to diagnose and treat. This chapter will focus on key aspects of the anatomy of the CMC region so that one can appreciate how and why these injuries occur. Further, salient aspects of the history, mechanism of injury, physical examination, and imaging necessary to make a prompt and accurate diagnosis will be reviewed. Classification systems designed to guide treatment will be highlighted. Nonoperative and operative indications and techniques will be discussed, in addition to the role of arthrodesis and alternative salvage options. CMC fracture-dislocations resulting in carpal fracture will also be examined.
