Abstract

Echocardiography has been recognised as a haemodynamic monitoring device in the critically ill patients. Although transthoracic approach provides discontinuous haemodynamic monitoring, it can be readily repeated at the bedside. In this chapter, the authors will describe how to use POCUS to estimate cardiac output non-invasively which would be pivotal in the diagnosis and management of cardiogenic shock. Myocardial contractility is one of the determinants of cardiac output. Hence, its assessment is paramount. There are different echocardiographic tools to evaluate left ventricular myocardial contractility. The authors will describe different methodologies, their main strengths and limitations. Starting with two-dimensional echocardiography (e.g., Ejection Fraction), Doppler assessment (e.g., dP/dT) and M-mode techniques (e.g., MAPSE). Finally, a section dedicated to regional systolic function in which latest recommendations for segmentation of the LV and different aetiologies for regional wall motion abnormalities will be discussed.
