Abstract

Post-acute care (PAC) and long-term services and supports (LTSS) represent critical venues of care for frail and elderly members of our society. Efficient and quality care that respects patient goals is the focus as the demographics of our population skew to greater proportions of elderly. The evolution of payment models from fee-for-service to total cost of care (value-based care) is increasingly holding providers accountable for the quality of care in global payment models. These will require new clinical strategies. In this chapter, we review components of post-acute and long-term care, operations, payment models, and strategies that will help post-acute/long-term service providers to be successful in advancing population health for an increasing frail patient population.
