Abstract
Defects of the central portions of the ear can either involve the convex shapes of the ear or the concavities of the ear (concha). For the concave areas, a postauricular island flap works well because the island pedicle continues to pull the flap inward, thus recreating the concavity. Therefore, the two main areas for use of this flap are in the concha cymba and concha cavum. More peripheral subsites such as the triangular fossa or along the antihelix can also be repaired in this manner, but will pull the ear posteriorly and may deform its shape.
