Dorsal Midbrain Syndrome: A Teenager with Diplopia, Headache, and Vomiting

Abstract

A 17-year-old man presented to the ophthalmology service with acute onset diplopia, headache, and intractable vomiting. He was previously in good health with only minor headaches until his presentation.

Video of eye movements. At baseline, there is an exotropia and bilateral eyelid retraction. There is limitation of upgaze. On attempted upgaze, convergence retraction nystagmus is noted (MOV 65605 kb)

Video of pupillary responses. There is a light-near dissociation such that the pupils do not have a substantial response to a light stimulus but with a near target, pupillary constriction is noted (MOV 59937 kb)
