Cyclic Vomiting Syndrome, Abdominal Migraine, and Chronic Nausea

Abstract

Cyclic vomiting syndrome (CVS) is a disorder of gut-brain interaction (DGBI) manifested by stereotypical episodes of nausea and vomiting. CVS shares both clinical and genetic features with migraine disorders and is often termed a migraine-equivalent condition. The distinction between CVS, abdominal migraine, and migraine headaches is based upon the predominant symptom during episodes. Functional nausea is an obscure and poorly defined entity of chronic, disabling nausea commonly overlapping with other DGBIs.

Despite improved characterization, recognition, and understanding of these disorders, without a delineated pathophysiologic cascade, they remain classified as DGBI (Table 38.1). Recent work has begun to expand the list of comorbidities and clinical phenotypes, while identifying pathophysiologic pathways and new therapeutic avenues. This chapter reviews the most recent data on mechanisms, comorbidities, clinical phenotypes, and therapeutic options with main focus on CVS.
