Abstract
Among the complex segmentectomies, the right apical segmentectomy (S1) is technically simpler than those of other complex segmentectomies. The location of vascular and bronchial structures can easily be approached using thoracoscopy. In addition, there are few anatomical variations in the vascular structures in the right S1 segment that should be always considered. These anatomical variations can be identified using preoperative 3D image reconstructions. Therefore, right S1 segmentectomy should be the initial complex segmentectomy procedure for beginners. This chapter describes details of the key aspects and tips for thoracoscopic right S1 segmentectomy, including the standardized technique to expose and dissect the segmental vessels and bronchus, and accurate division of the intersegmental plane.
