Post-Transplant Idiopathic Immune Complex Glomerulonephritis

Abstract

Membranoproliferative Glomerulonephritis (MPGN) is one of the important causes of end-stage kidney disease (ESKD), leading to kidney transplants. Moreover, the occurrence of MPGN in kidney allograft negatively impacts its survival. The reclassification of MPGN with immunofluorescence has transformed the way we diagnose and treat the condition in kidney transplant recipients. The optimal treatment of post-transplant idiopathic immune complex-mediated MPGN (IC-MPGN) is not established. In this chapter, we have attempted to highlight a classic case of de novo idiopathic IC-MPGN in a kidney allograft recipient. He achieved successful remission with rituximab and appropriate up-titration of immunosuppression.
