Abstract
Ocular ischemic syndrome (OIS) is an uncommon and underdiagnosed etiology of neovascular glaucoma (NVG). Its underlying pathophysiology may vary, but at its core is due to decreased perfusion of the eye. Classically, OIS is associated with stenosis of the ipsilateral carotid artery. Clinically, it may present with subacute decreased vision, amaurosis, ocular angina and an array of anterior and posterior findings including neovascularization (of the iris, angle, nerve, and/or retina), anterior chamber inflammation, and mid-peripheral retinal hemorrhages. Management is multidisciplinary and crosses subspecialties. This may involve retinal specialists for definitive diagnosis and additional therapeutic procedures as well as referral to other medical and surgical specialties for management of vascular risk factors and surgical revascularization. NVG may or may not be present in patients with OIS. Therapy specifically for NVG secondary to OIS has not been extensively studied and requires further research.
