Abstract

By promoting self-management and a healthy lifestyle for the patient, he or she can take control of his or her own life. The patient can give the disease a place in his or her life, increase the quality of life as much as possible, and prevent disruptions, limitations, and participation problems or other health problems. Self-management is a prevention-oriented, person-centered approach that is always tailored to the patient’s needs and based on shared decision-making. To change patients’ self-management, lifestyle, and health behaviors, caregivers need specific psychosocial and communication skills. Motivating is not easy. Nurses should focus on health outcomes, such as patient satisfaction, concordance, optimal lifestyle, and self-management. Health professionals must work with patients to achieve patient-centered care and treatment, with interventions that improve the patient’s health outcomes. To this end, health professionals must rethink their professional role and apply their professional knowledge and skills to the biopsychosocial situation of patients. This should lead to sustainable and shared decisions with patients.
