Abstract
In the previous step, the intervention was designed to target improved self-management and/or lifestyle behaviors. An intervention can only be effective if it is implemented properly. Much evidence-based interventions are not delivered as intended when the intervention was designed. Therefore, an implementation plan is created in intervention mapping step 5. The implementation plan includes the objectives you have for implementing the intervention. The implementation phase is about thinking about how to disseminate, implement, and embed the health intervention. The implementation plan describes the implementation strategy.
