Abstract
This chapter defines cultural competence and recovery and reviews important national and federal policy documents supporting its use in community psychiatry, as well as practical strategies on how to apply its principles. The rationales of culturally and linguistically competent care include (1) the increasing diversity of the US population especially ethnic minorities who are often underserved and experience disparities in psychiatric care, (2) health quality outcome goals of patient-centered and equitable care, and (3) the increased understanding of the complex intersectionality of cultural identity to include many cultural identity variables beyond race/ethnicity such as age, gender, sexual orientation, religion/spirituality, and socioeconomic status, among others. The concept of culturally competent care has also evolved to encompass linguistically competent care to underscore the importance of language in clinical care. The American Psychiatric Association originated the Outline for Cultural Formulation (OCF) in the DSM-IV. It was revised for DSM-5 along with the Cultural Formulation Interview, with 12 supplementary modules to provide clinicians questions to ask to obtain the information for the OCF. These tools for culturally competent care bring cultural issues into the assessment for diagnosis and treatment planning.
