Abstract

Identifying ST-elevation myocardial infarction (STEMI), acute coronary syndrome (ACS), and differentiating them from other mimics is a crucial skill for clinicians. Within this chapter, identification of a classic STEMI by anatomical locations is clearly delineated as well as STEMI criteria for sinus rhythm, left bundle branch blocks, and paced rhythms. Additionally, the mechanics of acquiring and identifying both right sided and posterior EKGs are highlighted. Left ventricular aneurysm, benign early repolarization, left ventricular hypertrophy, and pericarditis can each closely mimic a STEMI and must be properly differentiated from ACS. Other emergent percutaneous coronary intervention criteria to include DeWinters T waves as well as the inverted or biphasic Wellens syndrome T waves are also discussed.
