Abstract
Expertise in pharmacology and the clinical implications of medication use in frail older adults is essential to PACE programs. The pharmacist provides consultation with providers regarding mechanisms of action and key safety considerations for drugs used in this population. The Medicare Part D Drug Benefit is a major component of PACE operations and finances. To achieve success with clinical care, finances, and regulatory compliance with drug use, every PACE program must have a highly trained team of advisors with knowledge of the Part D program. This chapter describes the role of the PACE pharmacist and an in-depth review of the Part D benefit.
