Robotic Pediatric Renal Surgery

Abstract

The robotic surgery platform offers many distinct advantages in pediatric patients over traditional open or laparoscopic surgery given the robotic instruments endo-wrist abilities. The addition of fine-wrist movements provides surgeons the opportunity to dissect and suture within smaller working spaces, flattening the learning curve for urologic reconstructive procedures. Upper urinary tract robotic procedures are one of the most common applications of pediatric robotic surgery in urology, notably robotic pyeloplasty, ureterocalicostomy, heminephrectomy, and nephrolithotomy. These procedures require delicate dissection and reconstruction that the robotic platform makes possible within a laparoscopic approach. The robotic platform has been reported in patients as young as 6 months with success and typically only require 3–4 robotic ports. Herein this chapter, the general approach to upper urinary tract robotic procedures will be discussed as well as specific considerations on how to perform a robotic pyeloplasty, ureterocalicostomy, heminephrectomy, and nephrolithotomy in pediatric patients.
