Pathway to Convert a Potential in an Actual Organ Donor

Abstract

Paraná’s transplant system (Brazil) from 2011 to 2020 led a series of organizational strategies aiming to increase the number of actual donors for organ transplantation. Organ donation in the brain-dead scenario involves a pathway, which includes confirmation of brain death diagnosis, family approach, interview regarding organ donation, evaluation of medical contraindications, and donor’s care. When successful, it results in conversion; i.e., a potential donor is considered an actual organ donor. Each step is described with its inherent characteristics and challenges. Considering the total number of potential donors, reasons for non-conversion are divided in non-confirmation of brain death diagnosis (3% of cases), medical contraindications (35%) – with sepsis and cancer being the leading causes, 14% of family refusal, and 5% of cardiac arrest. In average, 58% of potential donor notifications does not result in an actual donor, illustrating the effort dispensed by the system to provide suitable organs for transplantation.
