Electrophysiologic Evaluation and Characteristics of Atrioventricular Accessory Pathways

Abstract

This chapter presents invasive electrophysiologic methods, definitions, and characteristics in diagnostic evaluating of non-decremental atrioventricular accessory pathways and inducible circus movement tachycardias. During electrophysiologic testing, atrial and ventricular incremental stimulation and extrastimulation during sinus rhythm and driving are used for (a) the diagnosis of bidirectional or unidirectional anterograde and retrograde (concealed) accessory pathways including their location, (b) the evaluation of the refractoriness and conduction properties of accessory pathways, (c) the determination of prerequisites and mode of induction of antidromic or orthodromic circus movement tachycardias, and (d) the demonstration of accessory pathway conduction in corresponding circus movement tachycardias by showing resetting of tachycardia, comparable activation sequences, and by changes in ventriculoatrial conduction times and tachycardia cycle lengths with ipsilateral bundle branch block.
