Abstract
This chapter presents multiple accessory pathways associated with complex electrophysiology and an increased incidence of antidromic circus movement tachycardia, atrial fibrillation, and ventricular fibrillation. Failure to recognize their presence is commonly responsible for failing catheter ablation. Multiple accessory pathways can be double to quintuple, left- or right-sided, ipsilateral or bilateral, overt and/or concealed, atrioventricular or combined with Mahaim accessory pathways. When right-sided, they are not uncommonly found in Ebstein’s anomaly. Disclosure and precise localization of all accessory pathways must be defined by a thorough electrocardiographic and electrophysiologic evaluation including mapping before catheter ablation. The electrophysiologic studies, mapping, and catheter ablation of various multiple accessory pathways are presented in this chapter.
