Preoperative planning is an essential prerequisite for revision surgery in order to determine the need for a transfemoral approach (extended trochanteric osteotomy) with its length of flap, the site of fixation and the fixation distance of the new stem, the type of new stem and component sizes to achieve this fixation, and the restoration of the correct leg length and center of rotation. An important part of the preoperative planning is also the exclusion of a periprosthetic infection. A diagnostic algorithm for this exclusion procedure and the process of preoperative planning (including the preparation of the corresponding planning sketches) are presented in this chapter.
