Cartilage Lesions and Osteoarthritis of the Hip and Ankle: Orthobiologics

Abstract

This chapter summarizes current strategies and evidence for the use of orthobiologic injectables for both hip and ankle cartilage lesions and osteoarthritis (OA). Injection therapies have become increasingly common both in the hip and ankle in particular for the treatment of OA, and some preliminary results also showed promising findings for the injective treatment of OLTs. The number of papers published about this topic is increasing, along with the availability of new products. The use of corticosteroids is the oldest injectable therapy for symptomatic OA but presents short-term and only palliative effects. Thus, new therapeutic options are being developed to solve the intrinsic limitations of corticosteroid injections, to reduce symptomatology and improve cartilage viability, protecting joints from further damage. Hyaluronic acid (HA) is a viscoelastic glycosaminoglycan constituent of synovial fluid and cartilage, while platelet-rich plasma (PRP) is a blood derivative rich in bioactive molecules which has also gained high popularity in the last years. More recently, injective cell-based therapies are also being explored for the treatment of hip or ankle OA. Though none of the available options demonstrated to be capable of reversing OA, they may have beneficial effects on symptoms and functionality and may contribute to delay the need for major surgeries.
