Orthobiologics for the Treatment of Plantar Fasciitis

Abstract

Plantar fasciitis (PF) usually responds to conservative treatment, but some cases are challenging to manage. New biological therapies have been suggested for multiple soft tissue problems and are gradually gaining the interest of the scientific world. Platelet-rich plasma (PRP), autologous whole blood (AWB), bone marrow aspirate concentrate (BMAC), and placental tissue extracts are new available strategies to manage PF. They fall under the umbrella name of orthobiologics: naturally derived substances from the human body having the ability to promote bones, joints, muscles, and tendons healing. Prolotherapy, instead, consists of a natural irritant, hyperosmolar glucose, injected into the soft tissues to cause osmotic desiccation and potential death of local cells, triggering a healing response. All these strategies can be used in PF treatment, replacing the current widespread use of corticosteroids and avoiding surgical release.
