Hybrid and Complex Aortic Aneurysm Endovascular Repair

Abstract

The evolution of endovascular aneurysm repair (EVAR) has resulted in the ability to repair aortic pathology, which previously had not been feasible due to anatomic challenges and procedural morbidity. For example, complete open surgery had traditionally been the “gold standard” for thoracic aortic aneurysm (TAA) repair. However, hybrid open and endovascular repair of TAA is now commonplace, negating the need for median sternotomy or laparotomy (in the case of thoracoabdominal aneurysms) and circulatory arrest/bypass in select cases, resulting in decreased procedural time and associated complications.

It has been demonstrated that up to 30% of patients with abdominal aortic aneurysms have at least one parameter that makes them not eligible for standard infrarenal stent grafts. These anatomic challenges include suprarenal aortic involvement, hostile proximal neck, involvement of visceral branches, and extension into the internal iliac arteries. The evolution of fenestrated EVAR (fEVAR) and EVAR using chimney/snorkel grafts (chEVAR) has led to an increase in the ability to treat these aneurysms endovascularly. This chapter will describe the advanced endovascular techniques employed to repair thoracic and abdominal aortic aneurysms.
