2-[18F]FDG PET/CT in Fever of Unknown Origin

Abstract

Fever of unknown origin (FUO) is characterized as fever of greater than 38.3 °C on several occasions during at least 3 weeks with indeterminate diagnosis after a number of required investigations. It is of utmost importance to search for potential diagnostic clues in a thorough history taking, physical examination, and the crucial investigations in order to diagnose FUO. In the absence of diagnostic clues, whole-body imaging should be considered to direct additional diagnostic tests. 2-[18F]FDG positron emission tomography/computed tomography (PET/CT) is a sensitive diagnostic modality for the assessment of FUO by enabling anatomical localization of focally increased 2-[18F]FDG uptake, thus directing further diagnostic tests to achieve a final diagnosis. 2-[18F]FDG PET/CT has some important advantages compared to conventional radiological imaging and has shown to be cost-effective in patients with FUO. This chapter provides an overview of 2-[18F]FDG PET/CT in FUO diagnosis.
