Abstract

This chapter provides a review of current understanding about CRPS pathophysiology. As more information becomes available about physiological processes that occur following an injury, several theories have emerged to explain the underlying processes which may predict the risk of developing CRPS and to help guide the choice of treatment. There is currently no unifying theory to describe the development of CRPS, though neural and immune mechanisms have been implicated. Some of the recent studies have examined molecular and metabolic changes at the site of injury. Other studies have described local and global inflammatory processes that lead to tissue changes and peripheral sensitization leading to the characteristic changes of CRPS in the affected limb. Further studies point to mechanisms within the central nervous system that may also play a role in CRPS. Ultimately it is likely a combination of these peripheral and central, neural, and immune mechanisms that are at play in development and maintenance of CRPS.
