Abstract

Perilunate dislocations (PLD) and fracture-dislocations (PLFD) are high-energy injuries that require prompt recognition and treatment. Missed injuries or inappropriate treatment can result in disability due to stiffness, weakness, and pain. Prompt recognition and closed reduction are indicated to avoid deficits associated with prolonged median nerve compression. The goal of surgical treatment is to reduce carpal malalignment, repair intrinsic and extrinsic wrist ligaments, and provide skeletal stability. Early intervention combined with postoperative physical therapy often results in good functional outcomes and the ability to return to a previous level of work.
