Abstract

Ultrasound guidance of percutaneous procedures offers a level of accuracy and safety beyond what is obtainable with landmark-guided injections, in which no imaging is used. Improvements in ultrasound technology have allowed greater detail in imaging anatomic structures, which has in turn enhanced the ability to visualize pathology and to select appropriate treatment. A greater level of patient safety can be obtained by using ultrasound guidance, when combined with a detailed knowledge of sonographic anatomy. Regional nerve blocks for percutaneous procedures are especially helpful when sensitive areas are involved, such as the palmar hand and plantar foot, and these can be easily performed with ultrasound guidance. Efficacy studies comparing ultrasound-guided to landmark-guided injections show variable results, but these studies are difficult to interpret, as efficacy is influenced by injection accuracy, diagnostic accuracy, and whether the type of injection used is appropriate for the condition being treated. This chapter also presents techniques for a variety of ultrasound-guided injections.
