Abstract

Periprosthetic joint infection (PJI) after megaendoprosthetic replacement of the distal femur as well as other sites represents a serious complication leading to prosthetic revision and often has a negative impact on reconstruction and limb survival. Orthopedic oncology patients have a recognized added susceptibility to PJI compared with regular joint replacements due to immunosuppression and larger implant lengths and surfaces. Therefore, prosthesis revisions need meticulous planning and execution with regard to surgical technique as well as knowledge of current treatment recommendations in infectious disease. As two-stage revision concepts are the favored gold standard, we present a case of periprosthetic joint infection of the distal femur treated by a two-stage approach. However, single-stage revisions or deviation from standard procedure seem warranted in a subset of patients when certain limited conditions and considerations are met, which is also debated in this case presentation.
