Managing Significant Varus and Valgus Malalignment During Total Ankle Replacement

Abstract

The demand for total ankle replacement (TAR) has continued to increase among patients and surgeons alike, as studies have shown higher survivorship for newer prostheses, with better improvements in function, and higher patient-reported outcome measures (PROMS) compared to ankles treated with arthrodesis (Harston et al., Foot Ankle Int. 38:1295–1300, 2017; Lee et al., J Bone Joint Surg Am. 100:835–842, 2018; Pugely et al., Foot Ankle Int. 35:207–215, 2014; Valderrabano et al., Clin Orthop Relat Res. 47–56, 2004). However, more than 50% of patients undergoing TAR have coronal deformity, which is the most common reason for additional concomitant procedures (Schuberth et al., J Foot Ankle Surg. 56:618–627, 2017; Dodd and Daniels, Foot Ankle Clin. 22:277–300, 2017). In this chapter, stepwise algorithms for addressing severe incongruent coronal (varus/valgus) deformity during TAR are detailed.
